What the document contains

1. To whom: the civil status registry division nearest to the applicant’s residence
2. The applicant or applicants: full name, date and place of birth, citizenship, passport or ID card, registered address, contacts
3. The title “Application” and a subtitle with the registration asked: birth, adoption, recognition of paternity, marriage, divorce, name change, correction or change of a record, annulment of a record, restoration of a record, or death
4. The request with the data the record needs: for marriage the surnames chosen after it, for a name change the current and new name and the reason, for a correction what is wrong and what is right
5. Attachments, date and the signature of every applicant

What to know before signing

• A marriage and a divorce by mutual consent are applied for jointly by both spouses: both are applicants and both sign.
• Divorce by mutual consent at the registry is possible when the spouses have no common minor children; otherwise it goes to court.
• This is a state form filled by the person: the application is drafted, never the certificate the registry issues.

What to attach

• Identity documents
• The receipt of the state duty, when a duty is due
• The documents the registration relies on, such as a medical certificate of birth, a court decision or the earlier certificate
