What the document contains

1. The title “Consent”, place and date
2. The parent giving consent: full name, date of birth, passport, registered address
3. The child: full name, date of birth, passport or birth certificate
4. The accompanying person, or “without an escort”: full name, passport
5. The destination country or countries, the period or dates of the trip, and the purpose when the person gives it
6. The parent’s signature and a note that it is certified by a notary

What to know before signing

• Write precise dates or a period, the countries and who accompanies the child; vague consent is refused by border and foreign authorities.
• Such consent is presented with a notarial certification.
