What the document contains

1. To whom: the Court of Appeal in civil matters (for administrative cases the Administrative Court of Appeal)
2. The appellant and the other participants with addresses
3. The judgment appealed: the court, case number and date
4. The grounds: which norms of substantive or procedural law were breached and how it affected the outcome
5. The request: quash or change the judgment, fully or partly
6. Attachments, date and signature

What to know before signing

• An appeal argues violations of law by the lower court, not just disagreement, and names each violation and its effect.
• The appeal deadline is not stated unless the person gives it.

What to attach

• The receipt of the state duty
• Proof that copies of the appeal were sent to the participants
