What the document contains

1. To whom and from whom: the head of the organisation by position and name, and the employee with position and unit
2. The request: to be released from the held position on one’s own initiative from the stated date
3. Optionally, a request for compensation for unused leave and for the documents the employee is due
4. Date and signature

What to know before signing

• The employee ends the contract by written notice at least 30 days in advance; 5 days is enough in the cases the law lists, such as illness or a breach by the employer.
• The notice may be withdrawn within three working days.
• If the leaving date is sooner than 30 days from the application, write it as you ask: the employer may agree.
