What the document contains

1. To whom: the head of the body by position (and name when known)
2. The applicant: name, registered address, phone, e-mail (for a company the name, tax ID, registered office and head)
3. The title “Application”, the facts in brief, then the request stated plainly and its grounds
4. An ordered list of attachments

What to know before signing

• An application must have the applicant’s name and surname (a company’s full name), address, the body applied to, the request, the list of attachments, the date and the signature.
• When a duty or fee is due, the receipt is attached, and when a representative files it, the power of attorney is attached.
• A body that is not competent forwards the application to the competent one within three days and tells the applicant.
